  • Claim Against City Form
    If you would like to submit a liability claim against the City, please fill out the above form and submit it with any documents pertaining to the incident. For more information on filing a claim against the City, please visit our

  • Do Not Knock Registry
    If you are a resident or business owner in San Carlos who wish to no longer receive visits from Solicitors on your premises, please fill out this form. Solicitors are required to review this registry prior to solicitation efforts and shall not solicit at any premises identified. 
  • Fair Political Practice Commission Forms
    Pursuant to the Political Reform Act and/ or certain local conflict of interest rules, local officeholders, certain City Employees and Candidates are required to complete various forms written by the Fair Political Practice Commission Forms (FPPC) on an annual, semi-annual or campaign schedule driven basis.   • Public Records Request
    Although you are not required to fill out a Public Records Request to obtain records, completion of the form makes the process more efficient. If you wish to complete the form, please download by clicking the above.
  • Public Speaker Slip
    If you would like to speak at a Council meeting, we ask that you fill out this short form. These forms will also always be available during a City Council meeting. Once submitted to the City Clerk, the information becomes public record. Please note that you are not required to complete this form or state your name and residence in order to speak.
  • Refusal of Consent Registry
    If you are a resident or business owner in San Carlos who wish to no longer receive unsolicited or unsubscribed handbills or literature of any kind from distributors on your premises, please fill out the below form. Distributors are required to review this registry prior to handbill or literature distribution efforts and shall not leave such handbills or literature at any premises identified on the list.
